- Fill out a new Google form.
- Inform the administrator of the verification SMS or Viber code during the phone call, which you will receive soon
- Receive a new document, print it and sign it.
- Send the signed copy within 48 hours to “Nova Poshta” (at the expense of the clinic).
- After processing the data, your new doctor will be assigned to you.

I want to sign a declaration with a new doctor, what should I do with the previous declaration?
