The process of concluding a declaration with a family doctor has several stages:
Contact the administrators of “Docos Medical” at any number listed in the contacts and inform them that you want to sign a declaration with a doctor.
The declaration is concluded during a personal visit, so arrange a free consultation time with your family doctor (the initial consultation at the clinic is for a fee).
The doctor will enter your data into the electronic system. An SMS with a code will be sent to your mobile phone. Report this code.
A declaration with your data will be printed from the electronic system.
Carefully check that all data is correct. If necessary, the doctor will make changes in the system and print the declaration again.
Sign two copies of the printed declaration. Take one with you, and keep the other at the institution.
The doctor will then confirm your declaration with his/her electronic signature and send it to the electronic system.
